- Application / Contract Review
- Initial Certification Audit (Stage 1 Audit)
- Assessment (Stage 2 Audit)
- Surveillance (Continual Assessment)

If clients require a physical application form, they may request it via email at: office@ccicert.com (or official CCI contact email if applicable).
2. Application / Contract Review
Upon receiving a complete application, the Certification Manager will initiate an application review process.
After the review is completed, a work order is issued to the client along with a copy of the client agreement for confirmation and acceptance.
3. Initial Certification Audit
A. Stage 1 Audit
PT CCI Kualitas Insani conducts a Stage 1 Audit after completion of the application review process.
A pre-assessment (optional) may be conducted prior to certification audit to evaluate the level of implementation and identify major gaps against the applicable standard. This helps clients prepare corrective actions before Stage 2 Audit.
Key points of Stage 1 Audit:
- Review of documented management system
- Assessment of readiness for Stage 2 Audit
- Identification of major deficiencies (if any)
- Preparation of Stage 2 audit plan
- Issuance of Stage 1 audit report to client
- Evaluation by Certification Manager
All auditors are required to declare conflict of interest before conducting any audit.
B. Stage 2 Audit
The purpose of the Stage 2 Audit is to evaluate the implementation and effectiveness of the management system against applicable ISO standards.
Auditors will collect objective evidence of conformity, and all nonconformities will be documented and discussed with the client's management representative.
At the end of the audit, a Closing Meeting is conducted where the Lead Auditor presents findings and makes a recommendation for certification.
Audit outcomes may include:
- Recommendation for certification
- Certification with corrective actions required
- Requirement for follow-up verification audit (for major nonconformities)
Corrective Actions
- Minor nonconformities require corrective action plans within 60 days
- Evidence of corrective actions must be submitted for review
Stage 2 Audit Coverage includes:
- Conformity to applicable ISO requirements
- Performance monitoring and measurement
- Legal and regulatory compliance
- Operational control of processes
- Internal audit and management review effectiveness
- Management responsibility and policy implementation
- Linkage between objectives, processes, and performance indicators
- Competence of personnel and documentation control
4. Certification Decision
All audit findings and evidence from Stage 1 and Stage 2 are reviewed before making a certification decision.
The Certification Body evaluates:
- Audit reports
- Nonconformities and corrective actions
- Confirmation of application review information
- Audit team recommendation
Based on this evaluation, PT CCI Kualitas Insani issues certification in accordance with its procedures.
5. Surveillance Audit
Surveillance audits are conducted at least once every year to ensure that the certified management system remains effective and compliant.
The first surveillance audit must be conducted within 12 months from the last day of the Stage 2 audit.
Objectives of Surveillance Audit:
- Ensure continued conformity to certification requirements
- Verify effectiveness of corrective actions
- Assess changes in the management system
- Confirm ongoing operational control and performance
- Evaluate continual improvement
- Ensure system remains appropriate to client operations
Surveillance audits are conducted on-site and may not cover the entire system at once but focus on selected elements.
6. Recertification Audit
Recertification is conducted every three (3) years to renew the certification validity.
The recertification audit shall be completed preferably before the certificate expiry date, allowing sufficient time for closure of any nonconformities.
The process includes:
- Full system reassessment (as required)
- Verification of continued conformity
- Review of surveillance audit results
- Evaluation of system effectiveness and improvements
Upon successful completion, a new certificate of registration is issued.
